As a professional with experience in geriatrics and case management, your AngelCare Care Manager understands the difference between normal aging and symptoms of illness. They utilize their knowledge and training to: assess their client’s needs; make recommendations and well researched referrals to other health care providers; and help the elderly and their families find safe solutions that fit their budget.

Comprehensive Assessment with Care Plan
Designed to look at all of the factors that affect an elderly client’s health, safety, and quality of life.
Includes an assessment of health history, psycho-social evaluation, functional evaluation and home environment safety analysis.
Home Environment/Safety Assessment
This includes an evaluation of safety factors (i.e. functioning smoke detectors, smoke alarms, removal of throw rugs, exposed wiring, etc.)
How effectively the older person is able to function within their home is also evaluated.
Special attention is made to prevent fall hazards and recommendations can be made for added safety protection such as the installation of: a shower bench; grab bars; raised toilet seat or toilet handles; adaptive hearing enhancing devices; wheelchair ramp; or wander guard equipment.
